How do you create a Customer Information entry page?

I need to create a page that allows the user to enter in customer information which includes 3 edit boxes:
User Name
Company Name
Serial Number
I have done some searching on this sight for a solution, but have not found one. This seems like such a common page for an install program that I feel like I am missing something really obvious. Can someone give me some pointers as to how to create such a page?
