Missing icon from shortcut

I installed my program on a win7 machine and the shortcut icons in start menu all programs seem missing. I tried the same install on a different win7 machine and it is fine. Does anybody know what the problem can be?

Do you install program for all user or only for current user?
Does installer have sufficient privileges?
